If you are planning on sending out a Mass Mailing to a list of recipients, you must fill out this form and send it back to us for approval. Failure to do so will likely cause most of your message being rejected.

4 - Each message will be required to have valid Opt-out information and the messages should only be sent to recipients who have requested your messages. The Opt-out must be clearly visible in all messages, including the provided example. We will not proceed if this requirement is not met.
5 a) - Can you send out an Opt-in email to each recipient under AOL, Hotmail and Yahoo requesting the recipient reply if they wish to continue receiving their messages?
5 b) - If a recipient does not respond, are you willing to remove that email contact from their mailing list?
5 c) - If you do not wish to proceed with the above Opt-in procedure for AOL, Hotmail and Yahoo (etc.) recipients are you willing to remove all these recipients from your mailing lists?
